Office Hours

Our office is open Monday through Friday from 8 a.m. to 5 p.m. We have operators that answer our calls until 4:30pm. We do have a number for emergencies should our patients need assistance after business hours.

Return Calls

The very best effort will be made to return all calls the same day however in many instances there is a 24 hour turnaround for telephone messages. Should your call require immediate attention please let us know.

Paperwork for the Physician

If you have any paperwork (such as FMLA, disability, etc.) that needs to be signed by the physician you will need to make an appointment. These forms typically require additional information from you.

Referrals

If you need an insurance referral to see a specialist please request it at least seventy two (72) hours before each appointment with your specialist, so we will have adequate time to obtain it. Doing so will ensure your referral is up to date and that your visit will be covered.

Medical Records

We maintain your records for 7 years after your last date of service; then they are destroyed. If you need a copy of your medical records, please fill out our Request for Medical Records form.
