Office Hours
Our office is open Monday through Friday from 8 a.m. to 5 p.m. We have operators that answer our calls until 4:30pm.
After Hours:
Our practice has a Cullman Internal Medicine physician on-call 24 hours a day, 7 days a week. Please call 256-737-2000 (CRMC) and ask for the CIM physician on call. If you have an emergency, please go to the nearest Emergency Room.

Return Calls
The very best effort will be made to return all calls the same day however in some instances there is a 24 hour turnaround for telephone messages. Should your call require immediate attention it would be best to make an appointment.
Paperwork for the Physician
If you have any paperwork (such as FMLA, disability, etc.) that needs to be signed by the physician you will need to make an appointment.
Online Appointment/Refill Requests
All online requests will be answered within one business day of submission. You can request an appointment online as well as request prescription refills. With prescription refills please note that the doctor puts a certain number of refills on each prescription so if your pharmacy says you are out of refills you may need to make an appointment to see the physician before he will give more refills. Please remember, with online requests, if you have an emergency it is best to call 911 or go to your nearest emergency room.
Referrals
If you need an insurance referral to see a specialist please request it at least seventy two (72) hours before each appointment with your specialist, so we will have adequate time to obtain the referral. Doing so will ensure your referral is up to date and that your visit will be covered. It is the patient's responsibility to obtain and confirm your referral.
Medical Records
We maintain your records for 7 years after your last date of service; then they are destroyed. If you need a copy of your medical records, please fill out the Request for Medical Records form. Due to the large number of medical records requested, it could take up to 14 days.
